The family can call the hotline number 0998-953-9437If Plan is active or fully paid, the deceased plan holder can be serviced equivalent to the Memorial Service Benefit.

  • First Call – release of the body from the place of death, assist in processing of the death certificate and transport the body to the mortuary.
  • Preservation, cosmetics and arrangement restoration, body placement in the chosen casket.
  • Final arrangement and viewing- provides facilities for the dignified & solemn viewing of the body (church, house, mortuary).
  • Interment – funeral cortege and provide appropriate coach for the deceased.

The beneficiary can visit to any CCLPI office and bring the following documents;

1. Claim thru Yearly Renewable Term Life Insurance;

  1. Original policy & or Certificate of Full Payment
  2. Death Certificate -original or certified true copy from the original duly signed by the local civil registrar.
    • If death certificate is photocopy, it should be originally certified by the local registrar of the place death occurred, bearing the official seal and the local civil registry number indicated at the upper right side of the death certificate.
  3. Proof of Beneficiaries:
    • If declared beneficiary is the spouse- Marriage contract (photocopy will do)
    • If declared beneficiaries are children;
      1. if children are below 18 years old; Birth certificate or Baptismal Certificate of children ( photocopy will do)
      2. Affidavit of Guardianship duly notarized (original copy )
    • For married daughters;

      Marriage contract ( to establish the use of another family name)

    • For children who are of legal age and still single

      Birth or baptismal certificates (photocopy will do)

  4. If Deceased is Single;

    If parents of the deceased are still alive, the proceeds of the insurance will be automatically granted to them. We will then require the Birth certificate or Baptismal Certificate of the deceased.

    If parents of the deceased are already dead, the next in line who is entitled as beneficiary/ies is/are the siblings of the deceased. The following documents will then be required;

    1. Birth or baptismal certificate of the deceased
    2. Birth or baptismal certificates of all single siblings (brothers/sisters)
    3. Marriage contract of all married sisters.
      • Should the siblings decide to appoint a representative to process and/or receive the proceeds of the claim, we shall require the following;
      1. Special Power of Attorney (SPA) duly notarized and original copy should be submitted;
      2. SPA should be supported by;
        1. Birth or baptismal certificates for all single sibling
        2. Marriage contract for all married sisters.
        3. Birth or baptismal certificate of the deceased.
      • Please check all the data i.e. spelling of names or dates. Should there be a discrepancy in the data of the documents, please attach an Affidavit of Two Disinterested Persons stating the true and correct data.
  5. If the deceased be it married or single has not declared any specific beneficiary, the legal hierarchy of beneficiaries will be followed;
    1. Surviving Spouse.
    2. Surviving legitimate, legitimated, legally adopted; and recognized natural children.
    3. Surviving illegitimate children without distinction.
    4. Surviving parents.
    5. Surviving brothers and sisters of the full blood.
    6. Surviving brothers and sisters of the half blood.
    7. Executors, administrators or assigned.
  6. If the declared beneficiary is only a live-in-partner without children, the following must be submitted;
    1. Certificate of no marriage from NSO for both the deceased and the surviving live-in-partner.
    2. Original copy of Affidavit of Cohabitation duly notarized.
    • Please note that only then the live-in-partner can be recognized if both parties have no legal impediment or both of them are single during their cohabitation and they were living together as husband and wife for not less than seven (7) years. Should there be any legal impediment the proceeds will then go to:
      1. The children borne out of their cohabitation.
      2. Surviving parents or sibling.

2. a. Accidental Death Benefit;(or murder /unprovoked assault;

  1. Original policy & or Certificate of Full Payment.
  2. Death Certificate -original or certified true copy from the original duly signed by the local civil registrar.
    • If death certificate is photocopy, it should be originally certified by the local registrar of the place death occurred, bearing the official seal and the local civil registry number indicated at the upper right side of the death certificate.
  3. Original Birth or Baptismal Certificate of Planholder and valid ID.
  4. Original Birth Certificate of Beneficiary (NSO issued) if claimants are children and valid ID.
  5. Original Marriage Contract of the Beneficiary (if claimant is the spouse) and valid ID.
  6. Original copy of Police Report signed by the investigator.
  7. For Medico Legal cases ( if there is no police report available):
    • Submit the autopsy report or post mortem examination report.
    • Submit a sworn statement or Affidavit of eye-witness.

2. b. For Vehicular Accident

  1. Submit driver’s license of the deceased ( if the driver was the deceased). If the driver’s license was lost at the time of accident, secure a certification from LTO stating the details of the license particularly the expiry date.

3. Claim due to Permanent Disability

  1. Medical Certification of continued disability for 180 days.
  2. Medical records (Abstract, Laboratory results, Medical History).
  3. Birth or Baptismal Certificate of Planholder.
  4. Hospital bill.
  5. Police Report if disability is due to accident.

**Standard forms to be included:

  1. Claimant’s Statement – to be filled-up by the beneficiary.
  2. Identification – to be filled-up by somebody who knows the deceased (third party).
  3. Attending Physician’s Statement (APS) – to be filled-up by the doctor who attended the deceased prior to death.
    • If the deceased died at home due to natural causes(illness), the following must be submitted instead Attending Physician’s Statement (APS):
      • Barangay Certification to attest to the occurrence of death at home or;
      • Narrative Report by the coop’s manager on the occurrence of death at home.
    • The above forms must be accomplished at least (2) copies each if amount involved is P250,000.00 and below. Higher than the mentioned amount, we shall require at least (3) copies each and must be duly notarized.

When all the requirements are complete and fully complied, the insurance benefit can be processed and the cheque will be released.

  1. Original policy and/or Certificate of Full Payment.
  2. Valid ID of planholder.
  3. Letter from the Planholder requesting for termination.
  4. Notarial fee of 200.

For reinstating lapse plan through updating:

  1. Application for reinstatement.
  2. All overdue premiums are subject to 2% penalty.
  3. Reinstatement fee of Php200.00.

For reinstating lapse plan through redating:

  1. Application for reinstatement.
  2. All overdue premiums are subject to 10% penalty.
  3. Reinstatement fee of Php200.00.

Scenario and Computation:
Updating Computation

  1. Contract Price: 50,000; GCP 900 Monthly
  2. Premium lapsed: 5 months
  3. 5 months x 900monthly premium = P4,500.00
  4. Penalty 900 x 2% x 5months = P90.00
  5. Reinstatement fee = P200.00
  6. TOTAL = P4,790.00

Redating Computation

  1. Contract Price: 50,000; GCP 900 monthly
  2. Premium lapsed: 5 months
  3. 1 installment of 900 monthly premium = P900.00
  4. Penalty 900 x 2% x 5months = P90.00
  5. Reinstatement fee = P200.00
  6. Finance charge: 900 x 8% x 5months; P360.00
  7. TOTAL = P1,550.00
  1. For the amendment of any or all of the following;
    1. Address
    2. Marital Status
    3. Beneficiary
    4. Relationship
    5. Birthdays
    6. Place of Birth
      Amendment Fees Required:
      • 1st Amendment – P50.00
      • 2nd Amendment – P100.00
      • 3rd Amendment – P150.00
  2. Request for changes of the following will not be charged;
    1. Correction of Names (Spelling, Middle Initials, etc.)
    2. Correction of Address (Street number, Spelling of places)

Due to Plan Transfer:

  1. Surrender of the assignor’s original policy.
  2. 2 Deed of Assignment and Transfer of Rights.
  3. Original/photocopy of any valid ID.
  4. New application form and signed by the assignee.
  5. Payment of notarial fee of Php200.00(if notarized).
  6. or Payment of 400.00Php for policy reissue fee(if not yet notarized).

LOST OF POLICY:

  1. Affidavit of loss, 3 copies.
  2. Latest copy of Planholder’s residence certificate.
  3. Payment of 200.00Php for notarize(if notarized)
  4. or Payment of 400.00Php for policy reissue fee(if not yet notarized).
